Primary Home Care (PHC)

Must be 21 or older or receiving PHC services through the Community Attendant Service (CAS) eligibility. PHC is a non-technical, medically related personal care service available to eligible adults whose health problems cause them to be functionally limited in performing activities of daily living. PHC service is provided by an attendant.

Community Attendant Service (CAS)

Adults and children with no age limit are eligible for CAS services. CAS is non-technical, medically related personal care service available for eligible adults and children whose health problems cause them to be functionally limited in performing activities of daily living. CAS service is provided by an attendant.

Family Care Service (FC)

Must be 18 years of age or older. FC is a non-skilled, non-technical attendant care service available to eligible adults who are functionally limited in performing activities of daily living. FC services are provided by an attendant and do require the supervision of a Register Nurse.
